ABSTRACT

Acute bacillary dysentery is a common health problem in the tropical countries. The routine method of microscopy culture and sensitivity (MC/S) for specimens sent to the medical microbiology laboratory, takes a minimum of 72hours for provisional diagnosis to be made and in developing countries where there is lack of modern technology for rapid diagnosis. Patients with acutebacillary dysentery suffer for longer period and this could be fatal. The purpose of this study therefore, was to determine the possibility of reducing the time spent on MC/S on stools from patients suffering from acute bacillary dysentery. A direct sensitivity test was carried out on carefully selected portion of blood and mucus stained stool  specimens from 100 patients  with acute bacillary dysentery sixty of the patients were in- patients in paediatric wards while forty of them were adults from out patients department, whose specimens were collected in the laboratory., fifty of the specimens yielded a heavy pure growth of Shigellaflexneri type 2,while the remaining fifty yielded a heavy pure growth of Shigellaflexneri type3 All the strains were sensitive to gentamici, neomycin and kanamycin, but resistant to ampicillin, erythromycine, chlorampheniocol and tetracycline. Al the patients were treated with 500mg/l of neomycine, mixed with mixed kaolin TD/S/7. Those on admission were discharge after three days, to continue their treatment at home. Direct sensitivity test can speed up treatment for patients with acute bacillary dysentery.

A cross sectional survey design was employed and the study aimed at exploring the level of awareness on Life Saving Skills (LSS), LSS practices been employed, barriers to implementation and strategies to promote effectiveness of LSS practice. A total sample of 154 nurses/midwives were recruited for the study at Murtala Muhammad Specialist Hospital, Kano. The result showed that 87.0% of the nurses/midwives were aware of LSS, Majority of the nurses/midwives reported numerous challenges  to implementation of LSS some of which include Shortage of skilled birth attendant (93.5%), Attitude of health care provider (79.2%), Inadequate equipment and supplies (81.8%) and Inadequate training for midwives (77.9%);  they also identified some of the strategies to promote effectiveness of LSSsuch as Provision of lifesaving skills equipment (84.4%), Training and retraining of personnel in life saving skills (88.3%), Accessibility of patient to the life saving skills services (97.4%).  The study concluded that there is a high level of awareness on utilization of LSS. Therefore, it is recommended that multilateral collaboration is needed in the study setting to overcome the limiting factors to utilization of LSS.

The paper examined the cost of illness among patients with Diabetic foot ulcer in General Hospital Minna and was a cross-sectional study involving 40 participants drawn from medical wards and diabetes clinic of General Hospital Minna. Questionnaire was used to collect data that was used to estimate direct medical, direct non-medical and indirect costs of DFU. The mean age for participants was 51.3± 9.1 years. The mean age for Males was 49.6.3±8.4 years while that of females was 58.9±8.7 years. Exactly 30% of participants had tertiary education while 17.5% had Secondary education and 45% do not have formal education and 7.5% had Quranic education. The average monthly income of participants was 25,187.5 Nigerian Naira however, 57.5% of participants had monthly income of 0 – 20000 Nigerian Naira. The total cost of diabetic foot ulcer is 4,526,300 Naira ($9839.8) per annum for the 40 participants. The total direct cost was estimated to be 4,162,400 Naira ($9,048.7). The total indirect cost was 363900 Naira ($791.1). From the findings it may be concluded that the treatment of diabetic foot ulcer in General Hospital Minna is relatively cheap, however still out of reach of average Nigerian. There is the need for the authorities of General Hospital Minna to strengthen the Nutrition department to meet the dietary requirements of diabetes patients and also the Social welfare department should be well funded to take care of the destitute and paupers who are diabetic but cannot afford the treatment to prevent complications.
